2/22/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. The client remained focused and ignored distractions in class while participating in morning stations. During afternoon observations, the client was observed observations, the client was observed the client interacted with his peers appropriately while handing out name tags for lunch. During afternoon observations, the client was observed the client followed directions on the first prompt while in resource. QMHP-C greeted the client upon arrival to the school setting. QMHP-C provided the client with positive reinforcement and encouraged him to remain focused and ignore distractions. QMHP-C collaborated with the client’s teacher During afternoon observations, the client was observed who stated he had difficulty keeping his hands to himself at recess. QMHP-C redirected the client and reiterated the importance of keeping his hands to himself. QMHP-C provided the client with positive reinforcement to continue to follow directions on the first prompt. The client responded well towards Therapeutic Day Treatment services as evidenced by positive feedback and participation. The client agreed to continue to remain focused and ignore distractions while completing station work. During afternoon observations, the client was observed observations, the client was observed the client continued to appropriately interact with his peers while handing out name tags for lunch. During afternoon observations, the client was observed the client was receptive to redirection and agreed to keep his hands to himself and to continue to follow directions on the first prompt. He also agreed to continue to track behavior on his self-monitoring chart.
